Primary Care
Our primary care team works to get to know you, your medical history and your immediate needs.
For your everyday health needs, turn to a primary care provider (PCP). Whether you need an annual checkup or are feeling under the weather, a PCP can help.
At Long Island Select Healthcare, we offer wellness services for patients of all ages at multiple locations. By establishing a relationship with our primary healthcare providers, you’ll be ready the next time you need a well check, an immunization, a video visit, a strep test or a referral to a specialist.

Complex medical needs can be challenging, but we have the resources and knowledge to help.
Everyday Medical Services for Health and Wellness
Going to the doctor shouldn’t be something that you do only when you’re sick. It’s just as important that you regularly visit your PCP when you are well.
How often you need to have a physical exam may depend on your age and your overall health. Most healthy adults should be seen at least once every other year. Children and seniors need to visit the doctor annually, and babies usually require multiple exams each year. If you have chronic medical conditions, your doctor may set up a special visit schedule for you.
Getting a routine well check gives you opportunities to build a relationship with your PCP. LISH is approved as an NYS Patient-Centered Medical Home (PCHM). Maintaining a strong patient-provider relationship is key to carrying out our responsibility as your PCHM.
During a wellness visit, we may:
- Measure your height, weight and other vitals
- Examine your eyes, ears and throat
- Run tests for diabetes, cholesterol and blood pressure
- Check for signs of cancer
- Ask about steps that you’re taking to stay safe and healthy and provide advice to improve your current plan
- Write prescription refills for maintenance medications
- Give you vaccines or booster shots
Staying on top of your immunizations is one of the best things that you can do to protect your health. Making regular visits to your PCP ensures that you’ll stay on schedule with flu shots and other essential vaccinations.
Treatment for Injury or Illness
Even if you do a great job of keeping up with your wellness care and maintaining your overall health, there may be times when you’re under the weather. Fortunately, LISH will be there for you in both sickness and health.
You may need to visit your PCP if you are experiencing:
- Fever
- Cough, sore throat or ear pain
- Persistent congestion
- Recurring headaches
- Stomach pain
- Skin rash
- Moderate injury
Of course, if you have sudden, severe symptoms or a major injury, the emergency room should be your first stop. For less-critical situations, though, it’s better to set up an appointment with your PCP than to visit the ER. Our doctors can provide a diagnosis and treatment, or they can refer you to a specialist who can help. Most of the time on the same day. It may be in person or through a telemedicine visit.
First Step for Specialist Services
LISH wants to be your Patient-Centered Medical Home. Together, we’ll form a team to advocate for your health and wellness. Both you and your PCP will be an integral part of this team.
We may bring in other specialists along the way. These may include:
- Dentists
- Behavioral specialists
- Allergists
- Dermatologists
- Neurologists
- Nurses
- Chronic care coordinators
We’ll customize your team members to your individual needs.
If you think that your healthcare requires a specialist’s input, start by talking to your PCP. Your doctor can put you in touch with other care providers who can help. Approaching your health needs in this way ensures that your care is coordinated and that all of your service providers are taking a team approach to addressing your wellbeing.
